Home Care

- An Overview

There is nothing like home for your loved ones, whether they are elderly, or those with disabilities. We all share similar concerns about their care and support and naturally want to enable them to remain in and enjoy their own home whenever possible and for as long as possible. That's where Your Health can really make a difference to the quality of life of those whom we serve.

Our services are tailored to individual needs and involve your loved one and where appropriate, the family and other health care professionals when they too are involved. Care can be provided from hourly to around the clock supporting your loved one; providing a service that will cover support with medication, housekeeping, meal preparations, sociable activities and personal care as well as loyal companionship.

All this is provided in the comfort and privacy of one's own home with familiar friends, family and possessions. For many of our clients, having someone to help them in their own home is their number one lifeline and can easily be the difference between them staying in their home or having to move into full time residential care. At YourHealth we realise that no adequate value can be placed on independence and the feeling that a person is still in control of their life. If we can help to maintain that for our clients, when we know we are doing our job right.

Types of home care

Respite Care

Respite Care is the provision of short-term, temporary relief to those who are caring for family members who might otherwise require permanent placement in a facility outside the home. It can relieve stress restore energy and promote the necessary balance in your life.

The reality is that there are times when even the most loving and dedicated of carers need a break. There can be any number of reasons for this; perhaps the carer is themselves unwell or has to be elsewhere, or maybe they are taking a well deserved holiday? This is where YourHealth can come in. Without respite care services, those cared for at home would often have no option but to enter into a residential facility. For those with dementia or learning disabilities this can be an extremely stressful and upsetting prospect; as well as for their carers too.

In these situations, YourHealth highly trained and experienced carers and support workers can offer our Home Care service on a temporary basis, be it from a couple of hours to cover a shopping trip, right up to a few weeks whilst the primary carer is away on holiday.

There should be no guilt attached to taking time away from a person for whom you care, but sadly there often is. Care giving is a difficult job and very few people are equipped to do it alone. YourHealth are here to help alleviate that, usually unfounded, guilt and you can be confident that that person is being looked after by someone who is not only professional, but really does care too.

Supported Living

Support with daily living can cover various tasks that you may have some difficulty with; for example feeding, bathing or dressing yourself, remembering to take your medication and shopping. Our support means that your life will be made so much easier and our services span over a wide range of duties.

Support with daily living

This may include getting up and dressed, preparing meals, assisting to the toilet, help with taking medication, collecting a pension, filling in forms and general companionship.

Bathing Service

We can assist with your daily washing and maintenance routine should there be help required in that area. YourHealth carers help with personal grooming such as shaving, trimming nails and hair care. Whilst these seem like simple tasks, we understand that for the less mobile they can be tedious and often impossible to do without help.

Day and Night Sitting

Daytime: We can visit in the daytime to provide company and support with daily activities.

Night time: We can attend on a “waking” basis to help you through the night or on a ‘sleeping’ basis where we provide peace of mind as we are present to help if needed.

Companionship care

As we get older, become fragile and less mobile, it is normal to feel in less control and lonely at times. One of the most rewarding aspects of home care support is the difference companionship can make. Our companionship services are tailored to support your choices and deliver the care you need to live your life how you choose.

Companion care provides a valuable social benefit by preventing isolation and helping to reduce depression from being alone. It also provides invaluable peace of mind to the person's family by offering information and respite for caregivers.

These non-medical services help make life more manageable. Companions are essential “eyes and ears” to family members. Most importantly, they act as a friend who listens and cares. YourHealth makes sure that you are never alone if you are worried about feeling withdrawn from society.

Dementia care

We provide care services to people who have memory deficits or are diagnosed with Dementia. We recognise the challenges that people living with Dementia and their families face. Our aim is to offer professional care to address these in a person- centred way. We aim to support individuals to cope with dementia in their homes, with support service that meets their needs. We are sensitive to our clients' life experiences and cultural preferences in our duty to help enhance their quality of life.

We can provide a range of support services that are person-centred and designed to reflect the individuality of the people we support. Our support and care services build on people's strengths and abilities to maximise their independence.

Families play a vital role in caring for their loved ones, and our duty of care to our clients ensures a healthy partnership with family members to provide a service that enables their relatives to continue to live independently at home. This partnership also involves active co-operation with the relevant key sector professionals.

Our overall goal therefore, is to provide a professional service that meets the full requirements of the sector and also fulfils the legitimate outcomes of our clients.

End of life (Palliative) Care

End of life care incorporates all elements of the daily life of a person nearing the end of their life. This includes personal care: strip and/or bed wash, mouth care, incontinent care, feeding, and aspects of domestic chores.

Agency Name recognises that the delivery of quality end of life care services to individuals, their relatives and friends require effective communication, empathy and understanding of the needs of the persons being cared for and the emotional expectations of their family members. Our care services ensure that these expectations are met through careful observation and monitoring and appropriate feedback.

Agency Name provides a dignified, sensitive and person-centred support, to meet the total needs of the client.

Live-in Care

A live-in care service provides around the clock home care for adults and the elderly who may need this level of support to continue living in their own home and community.

YourHealth will provide you with your own, dedicated live-in care worker who is always available in your own home to help you, day or night.

When a potential client or their loved one approaches us for live-in care, the first step is to find out as much as we can about that person, their interests, history and personality. Based on this information, the YourHealth team will produce a care plan, which is tailored to suit that person's needs and wishes.

Working in teams of 2 our carers provide round the clock care to your loved ones in the comfort and security of their own homes, either on a permanent or respite basis. We have carers specialising in a range of conditions including Dementia and Parkinson’s providing excellent round-the-clock care, without the disruption of moving into a care home.

We believe that continuity is the key and endeavour to carefully match the carers to the clients. We also develop and encourage our clients to be as independent as possible. Our carers are there to assist them in living the lives they wish to live, not to do things the way they believe they should be done. Choice is everything and YourHealth carers respect the choices and decisions of our clients in a safe and person centred way.

Along with your dedicated live-in carer, you and your carer also have the support of our experienced office team, so you always have the reassurance and comfort of knowing that you can always speak to someone from  YourHealth.
